COVID-19 visit information and procedures

If you’re new to the practice, please text or call 425-365-7964 to schedule an appointment.

I’m happy to announce that the office is open, and online booking is available for already established patients.

Your health and safety are always my top priority. With this in mind, you will notice some changes as we adhere to the interim guidelines of the CDC and Department of Health when you come for your appointment. We will be making the following temporary changes to our office procedures when you come to the office:

  • A COVID-19 Pre-Screening survey and a treatment consent form will need to be filled out before I can treat you. You'll be sent an online link forty-eight (48) hours before your appointment. If you can’t fill it out online, you will have to fill it out when you get to the office, before entering the office.
  • Prior to your visit, if you or someone you’ve had contact with are sick or have any symptoms of Covid-19, please call or text me ahead of time to reschedule your appointment.
  • Upon arrival at the office, please text or call me and I will meet you at the front door.
  • Check-in will consist of hand sanitizing, and checking your temperature with an infra-red, non- touch thermometer.
  • ONLY the patient that is being treated may proceed into the office. If a parent/guardian is needed at the appointment, this will be limited to one individual only. If multiple family members are in treatment and have an appointment, they may enter as well.
  • All people entering the office must wear a facemask, preferably a well fitting N95 or KN95 type of mask.
  • In addition to taking these precautions and steps for my patients, I will also be screened in the same fashion for signs or symptoms of COVID-19 in the morning and afternoon.
  • Rigorous disinfection protocols, along with an air filtration system will be implemented in the treatment room.
  • Please maintain a minimum of 6” distance from others while in the waiting areas.
